CVS Caremark Charitable Trust has awarded our Cerebral Palsy Medical Home Pilot Project. The Pilot Project will incorporate a unique, interdisciplinary team approach to care geared specifically to provide the best possible primary and specialty care to this subpopulation.  Under the leadership of a highly experienced chronic disease specialist pediatrician, the Cerebral Palsy Medical Home Clinic Team will, to our knowledge, be the first all pediatric trained interdisciplinary team consisting of  the following  disciplines: pediatric physiatrist (a physician who specializes in physical medicine and rehabilitation), physical therapist,  nurse care coordinator, clinical dietician and social worker.
